The Wimmera Primary Care Partnership (PCP) Health and Wellbeing Fund has been established to create a lasting legacy to support health and well-being improvements across the Wimmera Southern Mallee region. This fund, developed by the Wimmera PCP Committee of Management over the past two years, ensures that the impactful work of the Wimmera PCP will continue to benefit the community for years to come.

The Ballarat Foundation will manage the fund, including its grant-making processes, ensuring the long-term sustainability of health-related initiatives in the region. A local committee will be formed to oversee grant decisions, ensuring that community voices are central to determining how funds are allocated to meet the most pressing health and well-being needs.

The fund will provide financial support to charitable and non-profit organisations in the region, focusing on initiatives aimed at improving overall health outcomes, addressing mental health challenges, and tackling other critical health concerns.

The Wimmera Primary Care Partnership was a collaborative network of health and community service organizations that worked to improve health and well-being outcomes in the Wimmera region. The partnership focused on key areas such as chronic disease prevention, mental health, and social inclusion, fostering collaboration to create healthier and more connected communities.
